Policies

We accept most insurances. Plans presently outside our coverage include Cigna and Tricare Prime. Cosmetic procedures are not usually covered by insurance. As per your policy, you will be responsible for your deductible and co-pay. Services are payable at the time of your visit. We accept Visa and Mastercard, checks, money orders, and cash. Most insurances do not pay for office visits on the same day as a procedure. Also, we cannot predict the time or type of procedure that may be required prior to your assessment visit. So as to accommodate all our scheduled patients in a timely manner, we will schedule a separate visit for any necessary procedures.
APPOINTMENT TIME: We endeavor to keep a timely schedule. However, disease is unpredictable, emergencies occur, and procedures may run overtime. Dr. Jacobs will spend as much time as necessary when it is your  turn , for your health needs. Please keep your appointment. If you must re-schedule, call 24 hours in advance. Otherwise, $30 will be billed to your account to ease the blow to Dr. Jacobs’ ego.
